Client Service Agreement

This agreement outlines the terms under which Home Care Direct LLC provides non-medical home care services.

PARTIES:

  • Provider: Home Care Direct LLC, a non-medical home care agency
  • Client: The individual or family receiving services
  • Caregiver: The professional caregiver assigned to provide services

SERVICES PROVIDED:

Home Care Direct LLC provides non-medical home care services including but not limited to:

  • Personal Care (bathing, grooming, dressing, hygiene assistance)
  • Mobility Assistance (walking, transfers, physical activities)
  • Companionship and Emotional Support
  • Meal Preparation and Nutrition Assistance
  • Health Monitoring and Vital Sign Checks
  • Light Housekeeping and Laundry

SERVICES NOT PROVIDED:

The following services are NOT provided by Home Care Direct LLC:

  • Medical care or nursing services
  • Medication administration (reminders only)
  • Physical therapy or occupational therapy
  • Mental health counseling or psychiatric services
  • Any services requiring a medical license

CLIENT RIGHTS:

  • Right to receive care with respect and dignity
  • Right to be informed about treatment and care
  • Right to make healthcare decisions
  • Right to refuse services or treatment
  • Right to file complaints
  • Right to confidentiality and privacy
  • Right to accessible formats and language assistance

CLIENT RESPONSIBILITIES:

  • Provide accurate health and care information
  • Communicate openly with caregivers
  • Respect caregiver boundaries and professionalism
  • Provide a safe working environment
  • Follow agreed-upon care plans

CONFIDENTIALITY:

All information shared with Home Care Direct LLC is confidential and will not be disclosed without written consent, except as required by law.
